I've seen that video before. I'd like to think that what we know more about nutrition/fueling etc. now than back then, but we still see athletes end up like that during endurance events. It's not pretty when people finish their races like this, although I agree that it takes determination to finish in that kind of state. Thanks to that video, I was able to spot a marathoner at the Toronto Waterfront Marathon who was on the verge of collapse in the last kilometre. He didn't look as bad as that but he was on track for a 3:20-3:30-ish marathon and didn't appear to have any nutrition on him. When he did end up collapsing at the feet of a police officer, no one really did anything. Eventually, he got up again but kept swaying around and trying to run. I'm not sure how he ended up doing. People in that kind of state really need medical attention.

In the last half marathon I went in, there was a medical emergency that looked just like this, except that guy was hauled off.
