My time for my first 50 miler was 2 marathon times, plus 2 hours, kind of like Spaff. For a winter 50 miler, I'd count the cold as an adversary just like hills. I had it happen once badly when swimming a long swim in flat water. I associate it with loss of balance and orientation/proprioception. I routinely swim alone. I try to swim along the shore, or in a river where I'll always be close to shore. In a lake, deep water is best so you don't have to go out so far to get clear of the weeds.
